Nurse Practitioner: Permanent or Contract
A healthcare-technology organization in the Greater Lakewood Area is seeking Nurse Practitioners to join their team due to consistent census growth! This organization is a risk-based integrated care company that provides palliative care, home-based primary care, PACE (Programs of All-Inclusive Care for the Elderly) and programs to address social determinants of health.
They have a great reputation for serving patients with Medicare and Medicaid, who have complex care needs and an existing primary care provider.

Responsibilities:
As the Nurse Practitioner or Physician Assistant, you will:
Deliver clinical care, whether it be in-person at your patients home or virtually from your own home Provide compassionate, individualized, and holistic patient care including comprehensive primary care, urgent care, and chronic condition management Collaborate with our multidisciplinary care team to cooperatively deliver transitions of care support, social determinants of health interventions, and complex care management Schedule:
Monday - Friday, 3-4 days in the field to see 30 patients per week 1-2 days of the week will be administrative time working from home Weekly Interdisciplinary team meetings (can call in fully remote) Additional Perks/
