Social Services Assistant
Responsibilities:
Provide care coordination between patients and family members.
Provide direct brief clinical skills.
Monitor patients (in person, by telephone) for changes in clinical symptoms and treatment side effects or complications.
Participate in regularly scheduled (usually weekly) caseload care conferences.
Support psychotropic medication management prescribed by primary care provider, focusing on treatment adherence, attention to side effects, and effectiveness of treatment.
Facilitate treatment plan changes for patients who are not improving as expected in consultation with the team psychiatrist and primary care provider.
These may include changes in medications or psychosocial treatments or appropriate referrals for additional services.
Develop and maintain knowledge of applicable community behavioral health resources and programs for external referrals.
Exercise independent decision-making authority and expertise specific to assigned area of practice to enhance program development.
Attend appropriate interdisciplinary team meetings for the clinic, providing social work and behavioral health care perspective in clinic planning and treatment discussions.
Provides crisis intervention when necessary, linking the patient to specialized resources in the community for a higher level of care.
Provide consultation and cross coverage for other outpatient social work team members.
Perform other duties as assigned within scope of job.
